Jo Rycroft-Malone, PhD, is Distinguished Professor and Executive Dean of the Faculty of Health and Medicine at Lancaster University in Lancaster, England. Anita Kothari, PhD, is Professor at the School of Health Studies at the University of Western Ontario in London, Ontario, Canada.
